Hospital Support

We can help with arranging extra help by providing their competent carers to care for you or your loved one. A discharge assessment will be undertaken once they receive and accept the care request.

While the Service User is still in hospital, the hospital staff will contact Social Services to organise the discharge assessment. Social Services will in-turn contact us and the assessment can take place in hospital or at home. It does not matter if the hospital visit was planned or an emergency. The Service User will be involved in the assessment and a care plan will be developed with their participation.

The type of care received could be temporary which means it will be provided for a short while to help you get back to normal life after an operation or short illness. At times it will be ongoing which means it will be provided for a much longer period. Support provided could include help with shopping, preparing meals and personal care.
